Health Services / Immunizations

State law requires each student to be fully immunized against diphtheria, tetanus, polio, measles, mumps, rubella, Hepatitis B and varicella. The dose schedule is consistent with The Advisory Committee for Immunization Practices(ACIP). Measles, mumps and varicella immunizations must be acquired no more than four days before the child's first birthday. A Certificate of Immunization issued by a private physician or clinic is needed as proof. The certification must be signed and stamped by the physician. Copies of school records are not considered acceptable proof of immunization unless signed and stamped by a physician. Consistent with New York State law, students who fail to meet these requirements in a timely manner will be barred from attending school until all requirements are met.
